Preadmission Screening and Resident Review (PASRR) is a federal- and state-required process that is designed to, among other things, identify evidence of serious mental illness (SMI) and/or intellectual or developmental disabilities (ID/DD) in all individuals (regardless of source of payment) seeking admission to Medicaid- or Medicare-certified nursing facilities.

Challenge

  • To address security concerns regarding the current Quickbase platform, the decision was made to migrate the PASRR process and associated data onto a consolidated platform (from Quickbase to Salesforce) that would reduce the amount of redundant data entry and provide stakeholders secure access to PASRR data

  • The new system would need to posses: ​

    • Functionality for organizations to access a portal for submission of applications, viewing applicant data & documents, and the ability to transfer individuals from one facility to another.​

    • Security of data to ensure the right people see the correct data within the database and within a portal.​

    • The ability to accommodate nearly 100 organizations such as nursing homes, hospitals, and mental facilities.

Solution

  • Eigen X and ForHealthConsulting created an application for internal use and an application portal in under 8 months for a go-live launch

  • Developed and implemented a community portal that collected PASRR application information using FormAssembly and entered the information into Salesforce, which was then routed to internal user queues for review

  • Enhanced internal processes via automations and a focus on user efficiency.​

  • Implemented sharing rules and security to ensure that portal users saw only their individual's information from their facility to protect privacy rights

  • Created self-service registration functionality for facilities to add individuals to access the portal, after a vetting and review process managed within Salesforce

  • End-to-end process maps of the PASRR technical and business process

  • Migration of Quickbase’s PASRR system to an Enterprise-level platform, Salesforce, that met UMass Chan’s data security requirements and business needs

  • Implementation of a portal that enhances collaboration and workflow across the PASRR process
